Summary:
The Director of Social Services provides effective clinical services to include group, individual and family therapy as ordered by the physician.
This position is responsible for developing and implementing individualized treatment plans utilizing therapeutically recognized interventions and directing the overall clinical social and activity therapies.
ESSENTIAL JOB DUTIES:
To perform this job, an individual must accomplish each essential function satisfactorily, with or without a reasonable accommodation.
Collaborate with discharge planners and other community referral sources to assure provision of appropriate social services to patients admitted to the facility Interview patient, families or significant others to complete a thorough assessment; collect data pertinent to treatment and discharge planning within 72 hours of admission Provide individual, group and family therapy to address specific issues identified in the treatment plan Complete necessary documentation corrections as identified by the Compliance Officer within policy guidelines Audit active and closed charts for compliance Participate in multidisciplinary treatment team meetings weekly Conduct monthly in-service with the activity therapist and discharge coordinator Other duties as assigned JOB REQUIREMENTS:
Maintain knowledge of appropriate discharge and aftercare resources Maintain accurate and up to date knowledge of all relevant regulations and standards regarding the provisions of clinical services Operate as a member of a multidisciplinary treatment team and patient advocate Communicate professionally and effectively with other health care professionals, aftercare resources and family members Adhere to ethical and legal considerations of discipline practiced Collaborate with administrator and physician to ensure all relevant regulations and standards are met, including Medicare, state licensure requirements and policies and procedures Adhere to and respects all policies and procedures regarding anonymity and confidentiality of all patient records past and present to include any written or verbal communications regarding patients identify, address, and situation Function in a friendly, supportive, courteous, respectful, cooperative, and professional manner with patients, families, physicians, referral sources, visitors, coworkers, and management Promote, adhere, and monitor for a drug and violence free workplace Comprehensive knowledge of patient rights, characteristics of persons served and interventions Comprehensive knowledge of treatment plans, goals, and modalities that are patient specific Comprehensive knowledge and sensitivity in patient confidentiality, and cultural diversity Successful completion of required training Handle multiple priorities effectively Reliable transportation Required Computer Software/Equipment used:

Qualifications:
Knowledge/Skills:
An individual with a minimum of a bachelor's degree in social work or a bachelor's degree in a human service field including, but not limited to sociology, gerontology, special education, rehabilitation counseling, and psychology.
One year of supervised social work experience.

Complexity:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
